This is an opportunity to join a multi-disciplinary team, passionate about supporting young people and enabling them to reach their potential.
This post involves providing assessment and counselling to young people, who have been referred by their GP.

 

Working closely with the Social Prescribing Link Worker to provide a holistic approach to wellbeing. It will also include the supervision of Volunteer Counsellors.

Social Prescribing Link Worker

We are looking to recruit a Social Prescribing Link Worker to empower people to take control of their health and wellbeing, focus on ‘what matters to me’ and take a holistic approach, connect people to community groups and statutory services for practical and emotional support.

 

The role will support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners. This role will be surgery based for four days per week, and one day per week will be based at Base 51 head office.

Trainee Youth Support Worker

Base 51 is recruiting for the new role of Trainee Youth Support Worker through the Government's Kickstart Scheme.
